We, the requestor, wish to use said copyrighted work under the compulsory license provision of Section 115 of the Copyright Act relating to the making and distribution of phonorecords, audio tapes, or CDs of such work.

Upon doing so, we shall have all the rights which are granted to, and all the obligations which are imposed upon users of said copyrighted work under the compulsory license provisions of the Copyright Act after distribution of phonorecords, audio tapes, or CDs of the copyrighted work to the public in the United States by another person under authority of the copyright owner, except that with respect to phonorecords, audio tapes, or CDs thereof made and distributed by us:

1. We shall pay royalties and account to the Publisher quarterly on the basis of phonorecords, audio tapes or CDs made and/or distributed; and
2. For such phonorecords, audio tapes, or CDs made and/or distributed, there shall be a specific royalty of 9.1 cents for compositions with a playing time up to five minutes, and 1.75 cents per minute of playing time or fraction of a minute if the playing time is longer than five minutes; and
3. This license covers and is limited to one particular recording of the musical composition set forth above as performed by the following artist(s):
   
   on the following record label and record number
   

   This License does not supercede nor in any way affect any prior licenses now in effect respecting recordings of said musical composition; and

4. We need not serve or file the notices required by the Copyright Act.
